Sky card keeps unpairing itself

Hi, my sky card keeps unpairing itself. I can pair it each time this happens but surely it shouldn't keep happening? Is there a fault with the box or card?

Have you tried doing this via your box itself:

 

Make sure your Sky viewing card is correctly inserted into the box.
Press Services on your Sky remote
Press 0, 0, 1, Select (setup should now be highlighted)
Scroll across to New Install
Press Select twice
