Why have i now got ultimate tv add on?

We're customers on this forum so can't make any changes for you but help you understand what might have happened and then how to correct it.

I was offered Ultimate TV free for 4 months, then £4.50 pm thereafter, during a phone call, but I declined. Now I have an email from Sky adding this to my subscription. I want it cancelled, as I did not agree to this.
