How do I reclaim termination charges from previous provider after switching to sky?

I changed over to Sky broadband and TV from BT and was advised when changing over any early termination fees from BT would be refunded by Sky, up to a value of £200, if broadband and tv switchover.  I have no idea how to reclaim this cost, having now recieved my final bill from BT.
