Refund still not paid

Sky owe me one hundred pounds and I've been waiting a long 15 months. Multiple attempts to return via bank transfer / DD have failed without anyone telling me why. The cheque in the post never arrived. Surprise surprise!
They tell me a manager will call me in 9 days, despite everyone else telling me 'the manager' has sorted it. Every person I speak to blames the last and no one resolves anything. What to do next!?
