I'm keen to watch Clip My Horse TV, a subscription service on Sky Q but can't add app or mirror through Apple TV, is an HDMI cable the only way to connect my phone? If so where do I plug this in?

Your best bet is to get the official Clip My Horse TV app on a streaming device and attach it to your TV.

The app is available on multiple platforms, including tvOS on Apple TV boxes.
