You haven't said what you issue is but these are the packages for Netflix via Sky:

Ultimate TV includes the Netflix Basic plan (1 screen + SD) as standard
You can upgrade to Netflix Standard (2 screens + HD) by adding the HD pack
You can upgrade to Netflix Premium (4 screens + UHD/HDR) by adding the Ultra HD pack (requires the HD pack first)

If you have any issues upgrading then give Sky a call.

On sky q I want to upgrade to another screen so more than one person can watch

As per the info above, you need to add the HD Pack to your subscription.

How do you pay for Netflix?

Is this directly with Netflix or included with a Sky Cinema subscription?

If it's with Netflix directly then you have to upgrade with them using your Netflix account.
