Currently on sky hub max but wanting to change to my own modem to replace this.

what are they connection username and password to give my new modem connection?

Are you on full fibre fttp or part fibre fttc vdsl

and it's only vdsl

You'd need a router compatible with DHCP Option 61 and use the connection string abc12345@skydsl|abc12345

I would also note that some areas & routers can authenticate automatically using IPv6 PD if you enable IPv6 on the routers WAN side.
