Hi can I know where the document shows my name and devices imei number and make and model of my device and date that your device was first and last used with your sim card

I presume this is following a device theft or loss?

As per Skys website ~

"The proof of usage is your email with the IMEl number on it which you got when you first took the device. That is what insurance companies need. So you need to look for that, not proof of usage as in the bills."

So look for your credit agreement which will be in your message centre:

https://www.sky.com/mymessages

It may also be a good idea to register any loss/theft on the Immobilise website https://www.immobilise.com

Thank you sir for giving reply. No, i took my phone direct payment and I took insurance on Lloyd's app. In that app they are asking these document and they told please not that am itemised bill will not be accepted can you give me solution for these one please

@Tarunkumar The information provided by @Daniel0210 and @Mark39 is all sky will give. You will need to take this up with your insurance company with the documents Sky provides.
