A few things to try:

Open the Phone app.Tap the three dots in the top-right corner.Select Settings > Caller ID & spam.Toggle on See caller and spam ID

Go to Settings > Apps > Manage Apps.Tap on Phone (or Contacts & dialer).Select App permissions and ensure it has permission to access your Contacts and Phone/Call Logs.
