What needs to go in box with the pre paid label other than phone , cable and sim pin?

All you need to return is the phone itself so any sim you have you can dispose of this yourself and I'm not sure they are bothered about the cable to you can keep that as a spare as some mobile suppliers like Apple and Samsung no longer supply them with new devices.

It does but if you want to include a note with your name and account number then there is no harm in doing this if you are yet to send it off.
